By signing this document, you will waive certain legal rights, including the right to sue. PLEASE READ CAREFULLY.

I, , hereby agree that I am participating in Shift Event (hereinafter the “Event) offered by Emily King and Gina Keeping (hereinafter referred to as the “Shift Event”). I recognize that this event may require physical exertion that may be strenuous and may cause physical injury, and I am fully aware of the risks and hazards involved. I understand as well that personal injury, death, property damage or loss may be caused or contributed to by the NEGLIGENCE OR CARELESSNESS of others.

I, , hereby agree that I understand this event is being filmed and photographed and video and photographs will be used for marketing purposes, promotion and other forms of advertising. I hereby grant Emily King & Gina Keeping permission to use my recorded image, video clips, action photos, likeness and voice without payment or any other consideration. I understand that my image may be edited, copied, exhibited, published or distributed and waive the right to inspect or approve the finished product wherein my likeness appears.

Additionally, I waive any right to royalties or other compensation arising or related to the use of my image or recording. I also understand that this material may be used in diverse settings within an unrestricted geographic area.

By signing this release I understand this permission signifies that photographic or video recordings of me may be electronically displayed via the Internet or in a public setting. There is no time limit on the validity of this release nor is there any geographic limitation on where these materials may be distributed.

This release applies to photographic, audio or video recordings collected as part of the event. By signing this form I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby. I hereby release any and all claims against Emily King and Gina Keeping and its employees utilizing this material.

In consideration of the Event Provider allowing my participation and use of the facilities and grounds associated with the Event, I agree, on behalf of myself, my heirs, administrators, executors, assigns, personal representatives and next of kin, that:

1. I ASSUME AND ACCEPT, WITHOUT LIMITATION, ALL RISKS AND DANGERS associated with my presence at the Event, or participation in activities thereof;
2. I ACKNOWLEDGE that I am of sufficient physical health required for participation in all activities at the Retreat;
3. I WAIVE ANY AND ALL CLAIMS against the Retreat Provider arising from or connected, directly or indirectly, with my presence at the Retreat, or participation in activities thereof;
4. I RELEASE, WAIVE, AND FULLY DISCHARGE the Event Provider from any and all liability for any loss, damage, injury or expense that I, or my next of kin, may suffer or incur by reason of my presence at the Event, or participation in activities thereof, due to any cause whatsoever, INCLUDING NEGLIGENCE AND GROSS NEGLIGENCE ON THE PART OF THE EVENT PROVIDER, OR OTHER PARTICIPANTS, OR ANYONE ELSE, and I accept that this release covers and includes, but is not limited to, all unknown and unforeseen claims, injuries, damages and losses, and any consequences thereof;
5. AND I WILL INDEMNIFY AND HOLD HARMLESS the Retreat Provider from any and all liability for loss, damage, injury or expense suffered or incurred by me or anyone else in connection with my presence at the Retreat, or participation in activities thereof.

I HAVE READ THIS DOCUMENT THOROUGHLY. I UNDERSTAND THAT, BY SIGNING THIS DOCUMENT, I GIVE UP IMPORTANT LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE. I UNDERSTAND THAT THE RETREAT PROVIDER IS RELYING ON MY FULL RELEASE AND WAIVER OF ALL CLAIMS WHEN ACCEPTING MY PARTICIPATION IN THIS EVENT.
